The Children's Central Intake Unit is the gateway into our programs serving children between the ages of 3-17. The intake process begins with referrals via telephone or face-to-face contact and is accomplished by a comprehensive initial evaluation. Consumers and families are then referred to an appropriate service within the agency or to an outside agency.

TARGETED CASE MANAGEMENT

Adult targeted case management serves individuals who have serious and persistent mental illness with co-occurring conditions such as drug and alcohol abuse and other special populations. The goal of the services are to assist the consumer in obtaining resources and services that build on their strengths and meet their needs to help them to function at their highest level in the community. It is a partnership between the case management team and the consumer that focuses on recovery and the self-management of mental illness and life.

The case management team and the consumer plan, coordinate, monitor, adjust and advocate for services and supports that are directed toward the achievement of the individual's personal goals for community living.
 

     


DETAILS

All referrals for adult targeted case management services are received from the Targeted Case Management Unit of the Office of Behavioral Health. The services provided by Targeted Case Managers include assessments, service planning, linking and gaining access to services, monitoring service delivery, problem resolution, informal support network building, and use of community resources.
