The Children's Central Intake Unit is the gateway into our programs serving children between the ages of 3-17. The intake process begins with referrals via telephone or face-to-face contact and is accomplished by a comprehensive initial evaluation. Consumers and families are then referred to an appropriate service within the agency or to an outside agency.

TARGETED CASE MANAGEMENT (TCM)

The primary focus of the Children's Targeted Case Management Program is to link and monitor the appropriate services for our clients as recommended by a psychiatrist or psychologist. Our program is not clinical. The Children's Targeted Case Management Program provides contacts to each consumer by an assessment of their individual needs ranging from one time weekly to one time monthly face-to -face visits. When a client or a family member cannot be contacted, immediate follow up is provided and continued until the client's status is established. The case managers access, coordinate and monitor the resources and services available to consumers and their families throughout the community. The program also assists consumers and their families in obtaining and maintaining basic living needs and skills such as: education, employment, mental health necessities, housing, food, medical care, and recreation. The age range for Children's Targeted Case Management clientele is from 4 to 21 years of age. A client who has turns 18 remains eligible for services until high school graduation or until turning 21 years of age, whichever comes first.

All referrals for service come from the Office of Mental Health (OMH). A representative at OMH screens referrals and assesses the medical necessity of the client; after which a determination is made regarding the appropriateness for intensive case management services. Community Council cannot refuse any referral made by OMH; consequently, the program receives a monthly stipend to service all clients regardless of the current insurance benefits at the time of their referral. In turn, case management staff is responsible to establish or reinstate insurance benefits through The Department of Public Welfare which makes the client CBH eligible- a major program funder.
