It is the purpose of Community Council Health Systems to help provide a save and drug-free work environment for our consumers and our employees. With this objective in mind and because of the seriousness of drug abuse problems in today’s workplace we have instituted a drug free workplace.

Case Manager Supervisor
Bachelor's + Master's Degree
$50,000 - $55,000
The Case Management Supervisor will supervise staff providing targeted case management services to adult clients with serious mental illness.
ESSENTIAL & CORE FUNCTIONS
1. Provide supervision of targeted case management staff providing service to adult consumers.
2. Monitor targeted case management records to insure that all services being provided are documented and that all mandated paperwork is completed on time.
3. Insure that targeted case management services are being provided according to regulations and the expected direct service contacts made.
5. Develop constructive relationships with key staff both inside and outside of the agency who provide services to clients.
6. Monitor time sheets, expense sheets, vehicle usage, tokens and other resources needed to insure effective service delivery.
7. Submit reports, monitor unit projections and other duties as assigned by the manager.
8. Direct service and on-call back up as necessary.
