Case Management

Children's Blended Case Management

The Children’s Blended Case Management program helps connect participants and their families to the supports and resources they need—both formal and informal—to thrive at home, in school, and in their communities. Our team works closely with each family to identify goals, coordinate services, and provide guidance that promotes the participant’s highest level of functioning and well-being.

The ultimate goal of the program is to empower participants and families to access and utilize the supports they need independently, helping them build the skills and confidence to continue succeeding once case management services are no longer required.

Non-Fidelity ACT

The Non-Fidelity Act program helps individuals access and make full use of community supports, along with mental health and substance use services, to live a stable, healthy, and safe life in the community of their choice. Our team works closely with each participant to assess their needs, develop a personalized goal plan, and connect them to the agreed-upon services and supports.

Ongoing monitoring and guidance ensure that participants continue to receive the resources they need to achieve their goals, fostering independence, well-being, and a strong connection to their community.
